Founded in 1995, Intercede Health is a medical management company headquartered in Houston, Texas. Its services today include an extensive network of Hospital Care Programs, outpatient post-discharge and chronic care service centers, a wide range of consulting services nationwide, and transitional care programs, including at home support for Home Bound patients. The company has partnered with large national health systems, insurance plans, ACO's, and IPA's as turn key solutions covering large complex markets. Intercede Health does not see itself in a traditional “vendor” role. Its mission is too vital and important to simply do the basics.
